Ad Book Form - The Ad Book form should be used when speaking to businesses or individuals about placing an ad or message in the book.  80% of all ads sold go directly to the individual players account.  Any questions regarding the ad book can be directed to one of the Club Directors.  The Ad Book form is available here and on the Forms page.

Important: The ad book form, payment and a copy of the ad to be used must be in the possession of the ad book coordinator by March 4, 2012 to be included in the book.  Failure to meet the deadline will result in the ad not being placed in the book and the payment returned to the business or individual placing the ad.

• Rates for the ad sizes available are on the form.

• The business or individual placing the ad should supply the information to be used.  Camera ready art if available.

• Each page of the ad book is standard paper size (8.5" X 11")
